Fix content models

* Fix placing of definitions by introducing a new “content” content model,
  reflecting how they are parsed
* Merge TopLevelContent and BlockContent into FlowContent
* Add TransparentContent to reflect that links can’t exist in links, even if
  there’s emphasis or so between
