This PR adds some UI changes for facilitating MFA. The following features have been added:

  1. MFA token is automatically requested when a new user is created
  2. There's now a checkbox to allow users to request a new MFA token when they edit their profile
  3. The MFA setup information is provided in a "Setup Information" box on the main profile page that they must click to present.
