Supervisors will crash if a child fails to restart

In the unlikely event that a child starts during supervisor initialization, or when added to a supervisor, but fails to restart for the supervisor after a crash, the supervisor itself will crash because there is no match for the error condition. The expected behavior is the supervisor will continue to try restarting the child until the `intensity` is reached withing the allowed `period`.
