Basic tracing with OTel

Please add basic interception tracing. What should be traced:

• Whole interception
• Recording / audit logging (interception started / tool usage / prompts etc.)
• Upstream calls (calls to AI provider)
• Tool calls
• maybe more...

@pawbana we should also include the MCP server initialization (which connects to the servers and lists their tools). One span per server.
