I think the failures were effectively from the COM apartment marshaling layer, which likely tries to QI at some point and I suspect it doesn't do well when everything specifies E_NOINTERFACE. |

Try changing the ICustomQueryInterface implementation to always return NotHandled instead of Failed to defer back to the ComWrappers impl instead of failing every QI (which I suspect is what is leading to the failures).
