Specify which tables key cohort extractor functions pulls from in docs #114

This is definitely not urgent, but I was trying to compare a definition to identify care homes implemented in CPRD which used Read codes to search a series of tables (clinical, referral and consultation) and realised I wasn't sure which database tables contributed to each cohort extractor function.
If a lot of researchers come from CPRD to this, perhaps it would be useful to include text explicitly explaining that with_these_clinical_events searchers for code matches in the CodedEvent table, and that this covers clinical, referral and consultation fields (according to @alexwalkercebm).
Happy to write this up and open a PR if useful, or for someone to point out that this already exists and I've missed it/misunderstood...
